Pueblo Triple Aim has developed a variety of community-level data and goals to demonstrate how Pueblo County is moving toward becoming the healthiest county in Colorado. Our data fall into three categories to demonstrate the impact our partners are having on our community: County Health data, Triple Aim Data, and Intervention-specific Data.

County Health Data

PTAC uses data from County Health Rankings as one way to identify progress towards making Pueblo County the healthiest county in Colorado. We have developed five-year goals in the broad categories of Health Outcomes and Health Behaviors. For each area, we are measuring our progress relative to four comparison counties: Boulder, Mesa and Weld.

Health Outcomes (currently 51 out of 60 counties, 2016) – Goal to move to 43rd.

Health Behaviors  (currently 57 out of 60 counties, 2016) – Goal to move to 46th

Triple Aim Data

PTAC is using two Data in each of the three legs of the Triple Aim (Cost of Care, Patient Experience, and Population Health ) to identify progress in improving the health of Pueblo County citizens.

Population Health – Years of Potential Life Lost & Residents Reporting Fair or Poor Health

Patient Experience— – Clinical Care (Access and Quality) & Residents without Insurance

Cost of Care— – Illness Burden & Preventable Hospital Stays
